Incident report
Bell-Boeing V-22A Osprey crash near New Castle County Airport, Wilmington, Delaware, United States
minor1991-06-11
Summary
On its first flight after maintenance the aircraft rolled uncontrollably from a hover of about fifteen feet at Greater Wilmington Airport, the left nacelle struck the ground and the aircraft burned. Both crew escaped with minor injuries. The loss of control was later attributed to two of the three roll rate gyros having been wired with reversed polarity, so roll inputs were reversed. Aircraft number five of the pre-production batch was destroyed and flight testing was suspended.
