Bell-Boeing MV-22B Osprey crash near Arabian Gulf, Bahrain

Summary
Taking off from the amphibious assault ship USS Makin Island, the Osprey appeared to lose power. Two occupants jumped clear and the pilot landed safely after several dips in the water. One of the two who jumped was rescued; the other was not recovered and is presumed to have died. Marine Corps investigators concluded the aircraft had been started in maintenance mode, which left the exhaust deflector engaged and reduced engine power by about 20 percent. That errant mission system selection, together with a design flaw since corrected, deprived the engines of sufficient power for flight.
