Bell-Boeing MV-22B Osprey crash near Bellows Air Force Station, Oahu, Hawaii, United States
fatal2015-05-172 fatalities
Summary
The MV-22B held a prolonged hover in brownout conditions at the Marine Corps training area at Bellows, and an engine suffered a compressor stall after ingesting sand and dust. The aircraft came down hard and the fuselage caught fire. One marine was killed at the scene and a second died of his injuries three days later. Twenty two people were on board. The investigation faulted pilot decision making and an inadequate landing zone survey.
