Summary
On a night approach for a simulated embassy evacuation the aircraft descended too fast and too steeply and entered vortex ring state. The right rotor stalled at about 245 ft, the aircraft rolled over and struck the ground at Marana Northwest Regional Airport, and all nineteen on board were killed. It remains the deadliest V-22 accident.
