Airbus Helicopters H175 crash near Esbjerg Airport, Southern Denmark, Denmark

Summary

Following a maintenance inspection, the shift's inspection and tool control did not follow procedure and failed to identify a breaker bar with an attached socket left on the main rotor head. During the start sequence for engine number two, the bar and socket were ejected from the rotor head; the breaker bar penetrated the leading edge of a moving rotor blade and the socket landed on the apron. Eighteen people were aboard for an offshore flight. The investigation noted that human factors, partly stemming from an ongoing dispute with management over future shift patterns, organisational structure and maintenance procedures that engineers considered less than optimum, may have influenced the shift's decision-making and the effectiveness of the procedures applied.
