Summary
The helicopter was taking off from USS Peleliu and crashed shortly afterwards, killing all six on board. The cause was originally recorded as pilot error. Later accounts attribute it to rotor bearings that were not manufactured to specification, following a 1995 accident on Sikorsky's test ground.
