Summary
A Government Flying Service AS332L2 Super Puma lifting a full water bucket at Shing Mun Reservoir for firefighting lost its No 2 engine to an automatic overspeed shutdown. The captain ditched on the reservoir and all three crew got out unhurt. Investigators traced it to slippage of a freewheel shaft in the main gearbox, caused by faster than predicted wear during firefighting operations.
