Agusta A109S Grand crash near Valongo, Portugal

Summary
An INEM air ambulance A109S left Massarelos heliport in Porto for Paredes-Baltar in poor weather after an inter-hospital transfer, with two pilots, a doctor and a nurse aboard. Five minutes later the main rotor struck a radio broadcasting tower and its guy wires on Serra de Santa Justa, Valongo. The helicopter broke up and the wreckage fell on the eastern slope 384 metres beyond the tower. None of the four survived. Investigators found the flight was below regulated minimum altitudes, the crew were pressing on in marginal visibility, and the tower lacked night marking.
