AgustaWestland AW139 crash near Mondim de Basto, Portugal

An INEM air ambulance AW139 was landing at a quarry at Atei, Mondim de Basto, to collect a worker who had fallen, when dust caused brownout on short final and the crew lost sight of the landing site. The captain called for the landing to be aborted, but seconds later the main rotor blades struck trees at the edge of the site and the helicopter fell to the ground. All four on board escaped without injury. Investigators found a breakdown in crew coordination, loss of situational awareness, an incorrectly flown go-around and no specific brownout training.
