Summary
A Sikorsky S-76B operated by Island Express Helicopters crashed into a hillside in Calabasas, California in foggy conditions, killing all nine people on board. The passengers included NBA legend Kobe Bryant and his 13-year-old daughter Gianna, who were travelling to a basketball tournament at the Mamba Sports Academy. The NTSB determined the probable cause was the pilot's decision to continue flying under visual flight rules into instrument meteorological conditions, resulting in spatial disorientation and controlled flight into terrain.
