Summary
A Sikorsky CH-54A Tarhe firefighting helicopter, under US Forest Service exclusive-use contract, crashed into the Three Creeks Reservoir near Beaver, Utah while responding to the Widemouth 2 Fire. FlightRadar24 data showed the helicopter was airborne for approximately two minutes after departing Richfield Airport before crashing. Both pilots were killed; the aircraft was destroyed. The crash ignited a new spot fire in steep terrain, delaying access for investigators. The Widemouth 2 Fire had grown to over 106,000 acres at the time. The FAA confirmed the accident and the NTSB is leading the investigation.
