Sikorsky S-92A crash near Lagos, Lagos, Nigeria
The helicopter was on the ramp at Lagos about to request taxi clearance when the crew noticed a burning smell and saw ground staff waving urgently. The co-pilot saw smoke above his left window. There was no fire indication in the cockpit but the crew agreed there was a fire, carried out an emergency shutdown and waited for the rotor to stop before evacuating. Ground staff put the fire out with a standby extinguisher and it lasted only seconds. Nobody was hurt. A 115 volt cable loom had chafed and arced against a hydraulic pipeline, puncturing it, and the high pressure leak ignited on the hot surface of the right hand heat exchanger. Earlier serial numbers had been covered by chafing inspections in 2008, but no service bulletin covered airframes after 920072.
