Summary
The Puma was destroyed during a crew training flight, reportedly through rotor failure, killing both crew. The investigation is understood to have found a fault in the intermediate gearbox output bearing caused by a lack of lubrication — the gearbox casing had reportedly been produced without the hole through which oil is supplied to the bearing.
