Sikorsky S-92A crash near Hook Head, Ireland

Summary

During a search and rescue mission near Hook Head, Co. Wexford, an Irish Coast Guard Sikorsky S-92A (EI-ICG) crew located and winched two casualties from the water. During the winching operation, a 14-year-old girl slipped from the rescue strop and fell approximately 45 feet back into the sea. She was immediately recovered and winched up again. Both casualties were transferred to Waterford hospital. The girl died four days later on 10 December 2015. The AAIU investigation classified it as a serious incident.
