Bell 206B III crash near Opebi, Lagos, Lagos, Nigeria

The helicopter was flying from Port Harcourt to Lagos with a pilot, an engineer and a fitter on board, filed for two hours 45 minutes against three hours 15 minutes of endurance. It reported the field in sight at 12:11 and was told to report left downwind for runway 18L. That was the last transmission. It came down in a residential area at Opebi, about four kilometres east of the airport, and all three were killed. The causal factor was fuel exhaustion at low altitude and low speed leading to loss of control, with the decision to press on towards the destination rather than land as soon as practicable as a contributory factor. The pilot held the posts of director of flight operations, chief pilot and pilot in command simultaneously and was the operator's only pilot. His medical certificate had expired 22 days before the accident and his proficiency check four days before, and he lacked recent experience. The operator had no approval from the civil aviation authority to resume flying after the pandemic shutdown.
