Aerospatiale SA315B Lama crash near Tan Kwai Tsuen, Hong Kong

A Heliservices SA315B Lama collecting an underslung load at Tan Kwai Tsuen Quarry developed a left yaw that did not respond to pedal input and turned to an uncontrollable spin. The pilot made an emergency landing from 100 ft and the helicopter rolled onto its side. No one was hurt. The tail rotor pitch change rod had come loose because a cotter pin and secondary lock were not refitted during maintenance.
