Incident report
Bell 206L-4 crash near Okan field, offshore Escravos, Delta, Nigeria
minor2012-10-27
Summary
The helicopter was leaving the Okan offshore installation for Escravos when the main fuel pump failed. Wear on the external splines of the pump main drive shaft, which mate with the internal splines of the pump driver gear, caused fuel starvation, a flame-out and loss of engine power. The crew ditched about 100 metres from the Chevron installation. The helicopter landed hard on the water and the windscreen shattered. The floats kept it afloat, although the centre segment of the starboard float did not fully inflate and it tilted right. All four on board were rescued.
