Sikorsky MH-60R crash near San Diego, California, United States
Sikorsky MH-60R 166583, Seahawk 410, operated by the US Navy as a public use flight with two pilots and one non-flying crew member on board. Damage was substantial, to the left side of the stabilator. The two helicopters collided over the runway 26L underrun at Brown Field Municipal, San Diego, at about 17:50 Pacific time. Seahawk 410, the Navy MH-60R, was complying with the local controller's instruction to go around and overtook the HeliStream UH-60A from overhead as that aircraft was taking off. The UH-60A's main rotor blades struck the MH-60R's stabilator. Both crews landed and shut down, and nobody on either aircraft was hurt. The NTSB found the probable cause to be the failure of the crew of the second helicopter to maintain visual separation from the first while complying with the go-around instruction, with the night conditions and the absence of a safety alert from the controller as contributing factors. NTSB accident number WPR23LA045.
