Sikorsky S-92A crash near Black Rock, Ireland

Irish Coast Guard Sikorsky S-92A (Rescue 116), operated by CHC Helicopter, crashed into Black Rock island off County Mayo while providing communications relay for a rescue mission. Flying at 200ft in darkness, the crew failed to see the 282ft rock. The helicopter struck the island, damaging the tail, and tumbled into the sea. All four crew were killed: Captain Dara Fitzpatrick, Chief Pilot Mark Duffy, winch operator Paul Ormsby, and winchman Ciaran Smith. Ormsby and Smith were never found. The AAIU found Black Rock was in the GPS but not the terrain warning database. The report identified 12 contributory factors including inadequate charts and systemic oversight failures.
