Incident report
AgustaWestland AW139 crash near Umea, Sweden
serious2014-09-11
Summary
A Swedish Maritime Administration AgustaWestland AW139 (SE-JRM) was conducting confined area takeoff training with simulated one engine inoperative at Visby Airport, Gotland. The rate of descent became too high and the landing was hard, causing structural damage. No injuries. The SHK found the exercise was performed too far outside the exercise profile without the risks being identified. Contributing factors included no clear abort criteria and insufficient oversight from the aviation safety organisation due to staff shortages and turnover.
